Why Your Knee Pain May Not Actually Be a Knee Problem

The knee isn't always the source of the problem.

In fact, many cases of knee pain are the result of how the body moves as a whole rather than an injury to the knee itself.

Understanding the Body as a Connected System

Imagine trying to drive a car with one tire underinflated. The car will still move, but the other tires and suspension begin working harder to compensate.

Your body behaves much the same way.

Every step you take, every squat you perform, and every staircase you climb requires multiple joints working together. When one area isn't moving efficiently, another often takes on additional stress.

The knee is frequently caught in the middle.

 

The Knee: Built for Stability

One of the concepts I teach patients is known as the joint-by-joint approach.

While every joint contributes to movement, each has a primary role.

  • Your ankles are designed to provide mobility.

  • Your hips are designed to provide mobility.

  • Your knees are designed primarily to provide stability.

When the hips or ankles lose mobility, the knee often compensates by moving in ways it wasn't designed to. Over time, that additional stress may contribute to pain, stiffness, or decreased performance.

This doesn't necessarily mean your knee is damaged. It may simply be working overtime.

Three Simple Signs Your Body May Be Compensating

While a comprehensive movement assessment provides the clearest picture, here are three signs your movement deserves a closer look:

1. Your knees cave inward during squats.

This may indicate reduced hip strength, limited ankle mobility, or decreased movement control.

2. One leg feels less stable than the other.

Difficulty balancing on one leg can reveal deficits in stability, coordination, or neuromuscular control.

3. Your ankles feel stiff.

Limited ankle mobility often changes how your knees absorb force during walking, running, and lifting.

These aren't diagnoses—but they are valuable clues.


A Quick Test You Can Try at Home

Try this simple ankle mobility assessment:

  1. Stand facing a wall.

  2. Place one foot about four finger-widths from the wall.

  3. Keeping your heel flat on the ground, try to touch your knee to the wall.

Ask yourself:

  • Does your heel lift?

  • Can one side reach farther than the other?

  • Does the movement feel restricted?

If so, your ankle mobility may be influencing how your knee moves.

Remember, this is only one piece of the puzzle—but it's a great place to start.

Why Strength Alone Isn't Always Enough

Many people respond to knee pain by strengthening their quadriceps or wearing a brace.

While strengthening is an important part of recovery, it isn't always the first step.

If excessive muscle tension, limited mobility, or poor movement patterns are present, strengthening without addressing those issues may reinforce compensation rather than improve it.
